All replacement orders must be accompanied be a either a shipping fulfillment issue form, or a quality assurance form.


The exception is replacement order not resulting from an issue, such as customer requested exchanges or customer error. 


ALL QA FORMS REQUIRE A LOT NUMBER OR PICTURE!
